The authors describe a case of keratoconus decompensation after radial keratotomy (RK). Bilateral anterior chamber depth increase, increased central corneal thickness (800 µm) probably due to hydrops and hypertrophic scar in the right eye, and central corneal thickness decrease (350 µm) possibly due to secondary thinning in the left eye (ultrasound biomicroscopy and pachymetry) have been observed. Corneal topography, impossible in the right eye, shows a superior corneal ectasia (56D) in the left eye. A metticulous preoperative examination is suggested, and any surgical indication that leads to corneal fragility should be avoided.
